I would not alter the core intermediate image sizes. I can confirm this was fixed in WC 1.5.2 🙂
Check out wp_insert_attachment(). Takes an $attachment array of metadata and a $filename and inserts it into the media library. The Codex page has a good snippet so I won’t include one here.

Use the plugin Media Tags. It has lot of useful features, including bulk administration and roles management. It is translatable or translated too.
