Images sizes are only relevant for uploads in the media library. If you have assets in your theme then you need to include whichever sizes are needed for how they’re used in the theme. You can use wp_get_attachment_link(), function if you pass the size argument as medium you will get the medium size image link. https://codex.wordpress.org/Function_Reference/wp_get_attachment_link
This was due to having an inline <?php $post = get_post()?> After moving this out and making it global, images stopped duplicating.
