You don’t need any plugin to make it works. When you enabled Permalink, WordPress would automatically create a .htaccess file in your root directory, you need to make sure that the root directory is writable. Please take a look at this article and see whether you can solve it.

As the ticket says, input_attrs is not working for: checkboxes, textareas, and other non-fallthrough cases to have the wrapper you would have to create your own custom control and add the div in the render part, or go the hackish way with JavaScript.
