This might be a quick fix for your custom post type 404 error page. Navigate to Settings > Permalinks, and then click the Save Changes button. Then comeback to check if the links doesn’t give you a 404 error.

After changing permalink, getting 404 for one particular category
It’s just a guess, but if you go to settings -> permalinks and use the standard (ugly) permalinks with all the variables in the url it maybe will work. If it works, you probably don’t have mod_rewite installed on your server. And you can’t use the “pretty” url’s.

I’ve not reviewed your code, just a quick thought: Have you tried Going to Settings => Permalinks and just clicking ‘Save Changes‘? That flushes the rewrite cache.
