Short answer- there aren’t any. If you click the help button in the editor, then the hotkeys tab, you’ll see the available shortcuts for the current OS/Browser. In OS X the modifier is Command instead of Control, but there aren’t any shortcuts for headings.
Use an id and CSS selector for the element and align it in your stylesheet.

wp_default_editor(); will tell you the default editor – the last editor used. You can hook into the update/save post action and store this for the current post in the options array or somewhere similar. It’s used in wp-includes/class-wp-editor.php to define the default editor when loading TinyMCE in the post editor.
This <!– br–> does not do anything, most probably WP nulled it because it was broken, so you should use <br /> , in HTML mode.

The Tiny MCE is looking for a file: wp-includes/js/tinymce/langs/en.js I just checked the file and on my installation the file path is this: wp-includes/js/tinymce/langs/wp-langs-en.js I deleted the ‘wp-langs-‘ part of the file name ‘wp-langs-en.js’ and I seem to be working again.
You can use the StackOverflow Editor for Comments, using the WP Markdown Plugin. If you want another one, try TinyMCEComments or something like that. Didn’t test this Plugin – but should work fine.

If understood correctly, you are referring to the Quick Edit Reply action. There is no hook (a direct one at least) to hide that. It has to be CSS or jQuery.
