From quick look at plugin’s code it clearly has wpinfobox and wpinfobox-title IDs included in markup and only refers to them explicitly in CSS. If you are not content with its markup you can remove plugin’s filter and re-use it custom fields by your own code by analogue with the_box() method.
Try the Multiple Post Thumbnails plugin, http://wordpress.org/extend/plugins/multiple-post-thumbnails/. It will allow you to set a post type to have more than one post thumbnail, so you can set a front, back, index separately using the built-in handling.
I’ve explored both options, using custom fields and a custom taxonomy, in previous versions of my plugin, Posts 2 Posts. The latest version uses two custom tables.
You can use spaces and uppercase chars(periods, commas, and more) without worry. Only time you’ll see any problems is with register_globals on(strict array key names when enabled), but who has it enabled these days? Any of these should be fine as custom field names.
