Does the 48×48 thumbnail file exist? If not, you can use a plugin like Regenerate Thumbnails.

Problem solved – the image width was being limited by $content_width set to 640 in the functions.php file included with Bones WP theme. Hopefully this information will help anyone else struggling with similar problems.
A simple answer is that you can remove 2 unused sizes by enter 0 for their width or height. For example, you need only the small thumbnail 150×150 and full-sized image, so enter 0 for width and height of Medium and Large sizez.
