If I understood your question correctly: set your permalink structure as /%category%/%post%/ and from your mega menu create custom links for taxonomies categories and single posts.

The page not found error doesn’t mean that the template file is not found/used. Note that a URL returns a 404 status header (not found), then 404.php is used (or index.php if 404.php doesn’t exist). No. As far as the admin menu is concerned the taxonomies are always “below” the content they apply to. You will always need to directly manipulate the menu structure to change this.
If you are using WordPress newer than 4.5.0 get_terms() function is slightly different. Take a look at get_terms() definition. Second argument is deprecated and everything is passed through first argument.
I recommend using the Co-Authors plugin instead of building your own solution. That way, you use the existing WordPress user roles (Editor, Author, etc).
